**Purpose of the role**
The Service Coordinator role will coordinate services and lead practice at a site level, providing supervision, on the ground role-modelling, coaching and support for staff as well as supports to the clients, predominantly in the areas of health.

Achieve has been providing services to people with disability since the 1950s. Today, we are a registered NDIS provider offering integrated clinical, health, social and disability services in accommodation, community participation, supported employment and support coordination services in the Greater Sydney and in Northern Rivers regions in NSW.
**About the role**
Reporting to two Service Managers on site at North Ryde, this role closely oversees client health plans to create an empowering and enabling environment for people with disabilities and maximize opportunities for the attainment of health-related goals and overall holistic wellbeing. Through effective leadership, this role will support staff to meet the needs outlined and provide active support:
As a Service Coordinator your responsibilities include:
- Coordinating and overseeing the delivery of clinical services to our participants, ensuring their needs and goals are met effectively. Health related goals will be a primary focus of this role
- Collaborating with participants, their families, support teams and allied health professionals to develop individualized care plans that align with NDIS goals and guidelines.
- Conducting comprehensive assessments of participants' clinical needs and developing appropriate service plans accordingly.
- Liaising with healthcare professionals, therapists, and service providers to ensure seamless coordination and integration of clinical services.
- Monitoring the quality and effectiveness of clinical interventions and adjusting as necessary to optimize outcomes for participants.
- Providing clinical guidance, supervision, and training to support staff to ensure a high standard of care is maintained.
- Assisting with the development and implementation of policies, procedures, and best practices related to clinical service delivery in compliance with NDIS regulations.
- Collaborating with other stakeholders, such as NDIS coordinators, allied health professionals, and community organizations, to enhance participant outcomes and promote a multidisciplinary approach to care.
- Maintaining accurate and up-to-date documentation, including progress notes, incident reports, and service records, in accordance with NDIS requirements.
- Keeping abreast of changes in NDIS policies, guidelines, and best practices related to clinical services and ensuring compliance with all relevant standards.
**About you**
The suitable incumbent would have:
- A bachelor's degree in a relevant field such as nursing, social work, psychology, occupational therapy, or a related discipline is often required.
- Demonstrated experience and knowledge in providing clinical services, preferably in the context of disability support and the NDIS. A strong understanding of disability-related conditions, therapeutic interventions, and clinical best practices is essential.
- Demonstrated understanding of NDIS, principles of Active support and human rights-based approach to service delivery.
- Strong interpersonal and communication skills.
- Experience managing and supervising staff and client outcomes in multiple accommodation settings.
- Proficient Computer literacy - able to easily learn and use MS Office Suite and online systems.
- Practical experience in achieving community inclusion and ensuring services maintain appropriate.
