Skilled Nursing Care Specialist

We are seeking a highly skilled and dedicated nursing professional to provide person-centred, evidence-informed care that promotes identified health outcomes.
This position involves assessing patient needs and developing individualised care plans in collaboration with the healthcare team.
The successful candidate will be responsible for providing high-quality nursing care to patients of all ages, including children, adults, and older adults.
Assess patient needs and develop individualised care plans in collaboration with the healthcare team.
Administer medications, perform wound care, and apply dressings as required.
Monitor patient progress and report any changes or concerns to the healthcare team.
Current registration as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA).
Recent relevant experience in a clinical setting, as deemed appropriate by Western Sydney Local Health District (WSLHD).
Effective interpersonal skills, with a demonstrated ability to develop and maintain relationships with clients.
Excellent time management and organisation skills, with attention to detail.
Evidence of computer literacy and understanding of relevant health-related information systems.
