Job Description

As a liaison nurse, you will be working with diverse teams across health and community sectors to make a real impact on the wellbeing of children in Out of Home Care. You will lead care planning in a vital child wellbeing initiative and work closely with families, general practitioners, private healthcare providers, and other service providers to coordinate assessments and plans.


The role involves implementing health assessments and health management plans for children and young people entering Out of Home Care within Western Sydney Local Health District. This will involve working within a multidisciplinary approach with key stakeholders both internally and externally, including government and non-government partners.


As a clinical nurse specialist, you will have experience in assessing children and their families in relation to their health and social needs. You will demonstrate currency of practice and competence in this area, as well as high-level communication, liaison, networking, and negotiation skills.


Required Skills and Qualifications
  1. Current Registered Nurse Division 1 with APHRA registration
  2. Minimum of three (3) years post-registration experience appropriate to performing the role of a Clinical Nurse Specialist Level 2 for children and young people in out of home care
  3. Demonstrated currency of practice and competence in assessing children and their families in relation to their health and social needs, including an understanding of and commitment to Families NSW principles
