Homelessness Case Manager Position

We are seeking a skilled Homelessness Case Manager to join our team. As a key member of our staff, you will play a vital role in supporting individuals who are homeless or at risk of homelessness.

Key Responsibilities:

  • Provide case management support to individuals who are homeless or at risk of homelessness.
  • Develop and implement individualized plans to meet the needs of clients.
  • Work closely with participants to develop action plans to meet their individual needs.
  • Provide practical and personal support, crisis budgeting, information, advice, and community referrals.
  • Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as needed.
  • Advocate and assist participants to access housing and specialist services such as drug or alcohol, mental health, legal, domestic, and family violence services.

Requirements:

  • Certificate IV qualifications in Community Services or similar.
  • Able to work within a high-performance framework with accountability for KPIs on program delivery and outcomes.
  • Effective time management and organizational skills.
  • Strong communication and interpersonal skills.
