Job Opportunity: Lead Group is seeking an experienced Nurse Practitioner to work in a highly regarded aged care organisation.
About the Role:
- As a Nurse Practitioner, you will assess, diagnose, and treat aged care patients using telehealth technologies.
- You will collaborate with healthcare professionals to develop tailored care plans, monitor outcomes, and adjust plans accordingly.
- You will create evidence-based guidelines, educate residents and families on chronic condition management, and maintain accurate documentation.

Requirements:
- Current AHPRA registration and nurse practitioner qualifications
