Social Care Specialist

Job Overview:
- We are seeking a skilled and experienced Social Worker to provide comprehensive social services to patients, families, and carers within the Emergency Department.
About the Role:
- The successful candidate will be responsible for assessing and addressing social needs, developing care plans, and coordinating discharge planning with minimal direct clinical supervision.
- This includes providing crisis intervention services, staff and student supervision, policy development, and quality improvement initiatives.
Key Responsibilities:
- Providing trauma, counseling, case management, and bereavement support to patients and families.
- Developing and implementing policies and procedures for service improvement and providing training and education.
- Participating in quality improvement initiatives, service evaluation processes, and contributing to a consultative role within their area of specialty.
Requirements:
- Degree in Social Work with eligibility for membership of a relevant professional association.
- Proven experience in crisis intervention, staff and student supervision, policy development, and quality improvement initiatives.
- Excellent written and verbal communication skills, including report writing and high-level computer literacy.
