Helping clients achieve their goals is at the heart of this role, where you'll be supporting them in understanding and implementing funded supports within their NDIS plans. This involves connecting them with relevant services to meet their unique needs, while building their capacity to confidently manage these supports.

This is a client-focused position that requires strong interpersonal skills, creativity, and resourcefulness in problem-solving. You will work closely with clients and their families or representatives to empower them to make informed decisions about their support coordination needs.

  • Key responsibilities include accepting and actioning referrals for NDIS Support Coordination, partnering with clients to strengthen their ability to manage supports, and navigating the NDIS funding framework.
  • Additionally, you will respond appropriately when a client's needs change or escalate, and maintain strong relationships with internal and external stakeholders.

Our ideal candidate will have experience in case management or support coordination within the Disability sector, with a strong understanding of the NDIS framework and terms of business. Tertiary qualifications in a relevant field are desirable, but what matters most is your kindness, empathy, communication skills, and ability to work independently.

As a skilled Support Coordinator, you will play a vital role in delivering positive outcomes for our clients, which includes assisting them in accessing the right services to meet their individual needs.
