Support Coordinator Specialist

The key focus of this role is to empower clients by facilitating their understanding and implementation of funded supports within their NDIS plans, while connecting them with the services required to meet their needs.

  • Receiving and acting on referrals for NDIS Support Coordination
  • Partnering with clients and their families/representatives to enhance their capacity to manage supports within their NDIS plan
  • Navigating and effectively working within the NDIS funding framework and terms of business
  • Responding appropriately when a client's needs change or escalate
  • Building and maintaining strong relationships with internal and external stakeholders

This specialist role requires experience in case management or support coordination within the Disability sector, with a strong understanding of the NDIS framework and terms of business. The ideal candidate will bring kindness, empathy, communication skills, and ability to work independently while remaining connected to a supportive and collaborative team.

Key Responsibilities:
  • Proven experience in a support coordination or similar role within the NDIS framework
  • Strong interpersonal skills to build and maintain meaningful professional relationships
  • Creativity and resourcefulness in problem-solving
  • Self-motivation and drive to consistently deliver positive outcomes for clients
  • Mandatory qualification: NDIS Worker Screening Check, Police Clearance and WA Working with Children Check
