Support Coordinator for NDIS Participants

About the Role
We are seeking a Support Coordinator to join our team in providing high-quality support services to NDIS participants. As a Support Coordinator, you will work closely with individuals and their families to develop personalized plans that meet their unique needs and goals.
Key Responsibilities
- Conduct comprehensive assessments to identify participant needs and preferences
- Develop and implement tailored support plans that promote independence and self-advocacy
- Collaborate with healthcare professionals, carers, and community organizations to ensure seamless service delivery
- Maintain accurate records and reports to track participant progress and outcomes
What We Offer
- A competitive salary of $65,000 - $85,000 per annum, depending on experience
