Job OverviewWe are seeking an experienced Transformational Disability Support Specialist to join our team in Greater Western Sydney. This is a unique opportunity to work with a talented group of professionals who share your passion for making a difference in the lives of people with disabilities.
Key Responsibilities:- Work closely with participants and their support networks to develop personalized plans that promote independence and community engagement.
- Utilize evidence-based practices and social model therapy to identify and address barriers that prevent individuals from participating fully in their communities.
- Maintain strong relationships with interdisciplinary teams and habit coaches to ensure seamless coordination of support services.
- Provide person-centered support and planning to help individuals envision and achieve their goals, promoting a significantly improved quality of life.
To be successful in this role, you will need:
- A Bachelor or Master's degree in Disability (or equivalent) and eligibility for DEAI membership upon commencement.
- The ability to obtain an NDIS Worker Screening Check & Working with Children's Check.
- A valid driver's license and willingness to travel as required.

- A competitive salary range of $83,582 - $100,342, including annual leave loading and superannuation.
