Main ResponsibilitiesWe seek a skilled Support Worker who can provide person-centred care to help customers achieve their objectives. Key responsibilities include:
- Contributing information for the development of customer support plans
- Providing support with personal care, daily living skills, transportation, social activities, medication management, and other assistance as required
- Maintaining accurate and up-to-date customer records on a daily basis
- Fostering professional relationships and networks with customers, families, caregivers, and key stakeholders
To be successful in this role, you will need:
- Certificate III or IV in Disability, Individual Support, Aged Care, or Nursing
- Experience working in the disability, aged care, or nursing sector
We are looking for someone who possesses:
- The ability to work proactively, take initiative, and demonstrate a willingness to learn
- Strong communication, problem-solving, and conflict resolution skills
- Excellent time management skills, as you will be working in a fast-paced environment
- Ability to work under pressure and meet deadlines with flexibility
