We are currently seeking an experienced Nurse or Allied Health professional to fill a key role within our multidisciplinary Advice Coordination and Expertise (ACE) Inpatient Care Coordination team.

About the Position

As a core member of our team, you will play a pivotal part in facilitating access to community services, ensuring a safe and sustainable transition from hospital to home for patients.

You will work closely with patients, families, and other healthcare professionals to identify clients' needs and develop effective care plans, leveraging your expertise in complex discharge planning and assessment of individuals with complex needs.

Your input will be invaluable in guaranteeing that clients receive the right service mix to safely support care in the community, thereby enhancing their overall well-being.

Key Requirements
  • An appropriate qualification in nursing or allied health and eligibility for registration with the relevant body
  • A minimum of 3 years clinical experience in a relevant setting, including acute and community environments
  • Experience working in multi-disciplinary teams and a preference for collaborating with others
  • Skills in assessment, discharge planning, and care planning
