Transition Specialist

We are seeking a skilled Transition Specialist to join our team at Corrections Victoria. This is an ongoing position within a busy and supportive environment.
About the RoleThe Transition Specialist undertakes prisoner risk assessments, facilitates parole application processes, and provides support and services to assist prisoners with their transition into, through, and out of prison.
- Participating in the development, coordination, and delivery of prisoner risk assessment, parole application, and pre-release services within a prison location.
- Undertaking prisoner risk assessments and needs assessments, coordinating prisoner applications for parole, and contributing to the Case Management Review process.
- Supporting the delivery of reintegration programs at the prison location.
- Coordinating and facilitating the delivery of sessions and services, both individually and in conjunction with service delivery providers.
- Liaising with external providers delivering pre- and post-release services and referring individual prisoners to reintegration pathways, according to eligibility requirements.
This role requires collaboration with prisoners and prison staff, as well as liaison with community correctional services and adult parole boards. The successful candidate will have demonstrated understanding of transition and reintegration issues and needs of prisoners, ability to administer risk assessment tools, capacity to work collaboratively within a prison setting, and relevant tertiary qualification or skills and experience in human services, welfare practice, education, or related fields.
