Stride offers various roles within its multidisciplinary team, focusing on delivering comprehensive and integrated care services. As a Clinical Care Coordinator Flexible, you will contribute to the delivery of high-quality evidence-based clinical care for children and youth, emphasizing early intervention and support up to the age of 25.
The role entails addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
Key Responsibilities:- Contribute to the delivery of evidence-based clinical care as part of a multidisciplinary team
- Address service gaps for the 'missing middle' through enhanced accessibility and integration of primary mental health care, psychosocial support, and early intervention
- Must have Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR, AASW registration/eligibility as a Social Worker, OR, Level 2 ACA registration/eligibility
- Knowledge and understanding of theory underpinning evidence-based interventions and clinical practice, with demonstrated excellence in practical application in areas of mental health and alcohol and drug use
- Highly developed communication (including written) and interpersonal skills
- Ability to engage with people, advocate on their behalf, and establish/maintain effective relationships with staff, clients, families & friends, and stakeholders
- Experience using electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications
- Eligibility to apply/currently hold a Blue Card for paid employment
- NDIS worker screening clearance for paid employment
- Part-time opportunity with flexibility to work 4 days per week, or a 9 day fortnight
- Based in Rockhampton, QLD (with some flexibility to work from home)
