About the Role:

We are seeking a dedicated Specialist Family Violence Practitioner to join our Mackay-based team. This hybrid role provides place-based case management support across both refuge and outreach services, assisting adult and child victim-survivors who are currently experiencing or have previously experienced family violence.

  • Deliver trauma-informed, client-centred case management in line with standards and models of care.
  • Build strong, trust-based relationships with clients that encourage empowerment, safety, and participation.
  • Conduct initial and ongoing assessments of client needs, risks, and goals from intake to exit.
  • Develop and implement individualised case plans in collaboration with clients, ensuring their active participation.
  • Facilitate access to appropriate support services, community resources, and advocate on behalf of clients where needed.
  • Maintain accurate, confidential client records in accordance with procedures using internal systems.
  • Monitor and review case plans to ensure they remain responsive to changing needs or goals.
  • Identify and report risks and service gaps, escalating concerns to the Team Leader as required.
  • Contribute to service improvement initiatives and meet key performance indicators within required timeframes.
