Case Manager Role Overview

This is a highly rewarding opportunity to make a positive impact on the lives of individuals facing diverse and intricate challenges. As a skilled Case Manager, you will play a pivotal role in overseeing a designated caseload and providing holistic proactive support to clients.

Main Responsibilities:
  • Oversight of a designated caseload, encompassing initial referral, intake, and eventual exit processes.
  • Daily face-to-face client meetings, addressing emergencies promptly and ensuring positive outcomes through follow-up actions.
  • Development and implementation of collaborative individual support plans, incorporating interventions tailored to meet the needs and goals of each client.
  • Proactive engagement with support partners to gather feedback and implement strategies to optimize client support.
  • Delivery of wholistic proactive support, including accompaniment to appointments and provision of assistance during these engagements.
  • Active participation in block meetings, resident engagement activities, and new housing sites to promote awareness of support services and referral processes among residents.
  • Nurturing of networks and relationships with external support agencies and community organizations.
Key Requirements:
  • Degree qualification in Human & Social sciences (e.g., social work, mental health, community development services).
  • A minimum of three years' experience in case management, involving clients with complex needs.
  • Evidence of ability to engage and support individuals with challenging needs, along with experience in developing comprehensive client support plans.
  • Strong understanding of underlying causes of homelessness and complex needs.
