Community Support Worker Role

The Salvation Army's Homelessness Stream provides high-quality services to individuals at risk of or experiencing homelessness. Our approach focuses on person-centered care, case management, outreach support, financial assistance, and referrals to specialist services.

Our philosophy is centered around providing dignity, respect, and quality service to all individuals, ensuring everyone has access to a safe, affordable, and secure home.

We are seeking a dedicated Community Support Worker who will work closely with individuals in crisis to develop and implement personalized case plans that promote their best interests and ensure their safety and well-being.

The ideal candidate will possess strong relationship-building skills, be able to assess client needs, and provide ongoing support and guidance. They will also maintain accurate and up-to-date client records in accordance with TSA standards.

Key Responsibilities:
  • Establish effective relationships with clients to encourage self-determination and participation.
  • Conduct ongoing assessments of client issues, needs, and circumstances from initial intake to exit.
  • Provide risk assessment and flag concerns to the Operations Coordinator.
  • Develop individual case plans and collaborate with clients to address needs and goals.
  • Assist clients in accessing support services and establishing community networks.
Requirements:
  • Minimum Diploma Level in Community Services or Mental Health.
  • Current First Aid certification.
  • Valid driver's license.
  • Willingness to undergo a National Police check.
