We invite applications for a Service Coordinator to make a difference in the lives of people with disabilities. As a critical interface between our organisation and participants, you'll ensure a participant-focused approach to service delivery.

Empowering participants to live meaningful lives involves utilising a person-centred approach to communicate with participants and their families, identify individual goals and support needs, and coordinate the delivery of high-quality services.

Key Responsibilities:
  • Utilise a person-centred and active support approach to ensure consistently high-quality participant services.
  • Mentor, train, and advise colleagues to uphold ethical and professional standards in participant care.
  • Build and maintain strong relationships with participants, families, and diverse stakeholders.
  • Ensure service level agreements are met and exceeded, maximising participant satisfaction.
  • Coordinate intake and exit processes, maintaining accurate documentation and records.
Requirements:
  • Relevant tertiary qualifications in health, social services, or related fields.
  • Demonstrated experience in a similar role, with comprehensive knowledge and understanding of the sector.
