Service Coordinator

Main Responsibilities
  • Empower participants to live meaningful lives by adopting a person-centered approach to communicate with them and their families, identify individual goals and support needs, and coordinate high-quality services.
  • Innovate and collaborate with an organisation committed to innovation aligned with the NDIS expectations, working with internal and external stakeholders to achieve optimal participant outcomes.
  • Develop a rewarding career by contributing to a positive workplace environment and embracing opportunities for personal and professional growth as part of a dedicated team.
Requirements
  • Use a person-centred and active support approach to deliver consistently high-quality services throughout a participant's journey.
  • Mentor, train, and advise Area Supervisors and Senior/Support Workers to uphold ethical and professional standards in participant care.
  • Build and maintain strong relationships with participants, families, and diverse stakeholders.
  • Ensure service level agreements are consistently met and exceeded, maximising participant satisfaction.
  • Coordinate intake and exit processes, maintaining accurate documentation and records to support participant care.
