About the Role

  • Provide individualized rehabilitation and support to consumers, addressing their unique needs and goals
  • Develop and maintain strong relationships with homelessness and mental health service clinical case managers and other community organizations
  • Deliver high-quality services to consumers within their community settings

Your Qualifications

  • Relevant experience in a similar role or capacity
  • Knowledge of complex mental health and drug and alcohol issues, as well as homelessness
  • Excellent written and verbal communication skills, with the ability to work effectively as part of a team
