SDC Program Manager

With our unique model of care which places the resident’s needs as your number one priority, you will help create a home away from home for our residents. This role requires you to provide tailored care including personal support within the residential home. **About the role:** As the Manager for complex behaviour cottages supporting people living with dementia, you will be a core member of the Hammondville (Southwood) management team and work with a wide variety of people including the Residential Care Operations team, Local Health District, RNs, Carers, residents, resident representatives, and the multi-disciplinary health team to ensure resident outcomes are met. **Essence of the role:** • Advocate for residents to ensure their care is person-centred and focused on their strengths, capacities and promotes their well-being at every stage. • Facilitation of clinical care delivery by the Registered Nurses and Specialised Carers • Hands-on modelling, coaching and mentoring in clinical and complex behaviour management areas and processes • Consultative in case management and clinical decision making • Clinical oversight and professional competence as a Registered Nurse with experience in complex dementia-specific care • Support and investigate clinical/resident incidents as required • Communicate effectively with all stakeholders, both within HammondCare and external partners to see the best outcomes for the residents. **To be successful in this role, you will have:** • Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team and effectively work with other staff.• Strong negotiation and conflict resolution skills. • Significant experience operating as a consultant or specialist nurse preferably within Aged Care • Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team and effectively work with other staff. • Strong leadership and influencing skills. • Excellent analytical skills. • Strong administrative and computer skills. • Tertiary qualifications in specialised dementia and/or mental health service nursing for older people. **Benefits - What are we offering in return for your expertise and experience.** • Market-leading remuneration • Salary packaging; $15,900 annually for living expenses, plus up to $2,650 each year for entertainment benefits. • Additional training & career development opportunities • Leave benefits – purchased additional annual leave. • Reward and Recognition Program • Fitness Passports: Full facility access can include gyms, group classes and pools for yourself and your family. • Free independent, confidential counselling services, wand wellbeing resources available through our Employee Assistance Program (EAP) for you & your family. • Exclusive discounts at MyCar; Servicing, Brakes, Repairs, and Tyre Care Plans • Exclusive commercial discounts for JB HI-FI, The Good Guys, Optus, Dell and Samsung For more information about the role please enquire to Mitch Jeffs: mjeffs@hammond.com.au HammondCare is committed to creating an inclusive workplace that promotes and values diversity and inclusion.
