Permanent Part time opportunity - 72 hours per Fortnight

Role Overview:

Join a team committed to providing direct customersupport and exceptional experiences daily, with wellbeing at the core of ourmission.

In this role, you will:
Provideperson-centered support to help customers achieve their goals andimprove their wellbeing.
Collaborate with support networks and the broader community to enhance customerindependence in areas such as living skills, community engagement,financial and health service access, social activities, personal care,employment, education, and training.
Report to the Program Experience Lead (or Service Delivery Manager,depending on the program) and work collaboratively with the wider team.
Supportand supervise employees at a lower level as needed, and communicateeffectively with other New Horizons teams.

Key Responsibilities:
Assistin resource planning, coaching, and mentoring support workers.
Promoteand demonstrate safe and healthy work practices.
Supportchange initiatives and model positive behaviours.
Participatein team meetings and share information respectfully.
Utilizeresources efficiently and record customer information accurately.
Coordinatedaily support and activities for customers.
Developand review individualized support plans.
Enhancecustomer experience through innovative solutions.
Maintainconfidentiality and professional relationships.
Ensurecustomer records are up-to-date.

You'll also bring to the role:
Relevantqualifications (Cert 3 or 4 in Disability Support or equivalentexperience).
Technicalliteracy in Microsoft Office and reporting systems.
Supervisoryskills and basic HR management knowledge.
Physicalability to support customers, including lifting up to 10 kg.
ValidNSW Driver's C class license.
Current Police Check (not older than 3 months), NSW WWCC, NDIS WC
