Our Care Advisor is responsible for working in partnership with our clients in receipt of CHSP & STRC funding. The main goal of our Care Advisor is to meet the objectives of the program with a focus on client choice.

**Main role responsibilities include -**
- Assisting people to remain living at home for as long as possible
- Enabling clients to have choice and flexibility in their care
- Support clients in decision-making
- Being responsible for and sensitive to client customs, beliefs and background
- Wellness and enablement
- Maintenance of independence and continuation of participation within the community
- Maintain up-to-date records in client files, assessments, care plans and documentation
- Prioritise referrals
- Maintaining client budgets
- Assessment and identification of client goals
- Liaising with service providers

**What you will bring -**

A relevant record of study related to Health, Aged Care or a similar industry is desired.
- Experience in care management, ideally in Aged Care.
- Current Victorian Driver’s license
- Current Police Check & NDIS clearance or willingness to obtain
- Ability to work within budgetary KPIs.
- Understand and promote a client centred approach.
- Covid-19 vaccinations (3 doses) and willingness to obtain a flu vaccination
