**Key Responsibilities**:

- Develop personalized support plans in collaboration with participants, focusing on their individual goals and aspirations.
- Provide direct assistance to participants in various daily activities, including personal care, social engagement, and skill development.
- Facilitate community integration and participation by organizing and accompanying participants to various events and activities.
- Collaborate with families, carers, and other professionals to ensure holistic and coordinated support for participants.
- Maintain accurate and up-to-date documentation in compliance with organizational standards and regulatory requirements.
- Participate in ongoing training and professional development activities to enhance skills and knowledge in disability services.

**Essential Criteria**:

- Diploma qualification in Disability Services, Community Services, or a related field.
- Demonstrated experience in supporting individuals with disabilities, including the development and implementation of individualized support plans.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with participants and their support networks.
- Strong organizational abilities and the capacity to manage multiple tasks efficiently.
- Commitment to promoting social inclusion and advocating for the rights and dignity of individuals with disabilities.
- Valid driver's license and access to reliable transportation.
- Current First Aid Certificate and Working with Children Check.

**Salary**: $70,000.00 - $90,000.00 per year
