**About the Role**

Coordinate the initial entry process for clients that are referred to our Aged Care programs nationally. You will be the first point of contact for all calls received by our current Aged Care Clients across the region. In some circumstances, you will be able to provide immediate advice and information as per LWB policy and procedure. You will coordinate matching and or/service responses that are appropriate to the program and service allocation. This is a 6 month contract supporting our National team, which can be based from any of our local offices with hybrid work from home arrangements.

**Key Responsibilities**
- Receive initial requests from the funding body/ customer or stakeholders regarding a service/ placement request
- Collect and document information relating to the client in order to assist with placement matching and service allocation.
- Where appropriate, escalate or triage calls
- Provide feedback to the Aged Care Client Services Team
- Log all calls and query types for record keeping purposes and analysis
- Comply with all Life Without Barriers policy, code of conduct, procedures and practices, external funding body requirements and legislation.

**Skills & Experience**
- Demonstrated experience in relevant area / industry
- Strong interpersonal skills and the ability to deliver client service advice
- Strong Communication skills
- Demonstrated experience in a customer services focused role
- Understanding of social welfare sector including Disability, Aged Care or Mental Health Services in a client facing or administrative position
- High level written and oral communication
