The Role: Support Coordinator

Casual to Permanent Full-time

As a Support Coordinator at, your role is crucial in assisting participants with National Disability Insurance Scheme (NDIS) plans to achieve their goals and enhance their independence by connecting with communities and accessing diverse support services.

Essential Requirements:
1. Allied Health qualifications or minimum Certificate IV in Disability, or equivalent practical experience.

2. Demonstrated commitment to the safety of children, young people, and individuals with disabilities.

3. Excellent time management, administration, and delegation skills.

4. Well-developed interpersonal skills, including the ability to explain NDIS processes in plain English.

5. Attention to detail and proficiency in Microsoft Office Suite and client information management systems.

6. Ability to work independently, prioritize tasks, and take initiative.

7. Current NDIS Worker Screening Check, Working with Children's Check, and first aid certificate.

8. Current driver's license and willingness to use own comprehensively insured vehicle.

Desirable:
1. Experience in an NDIS support coordination role.

Key Responsibilities:

- Support NDIS participants in coordinating their plans, negotiating services, and connecting with community and mainstream supports.
- Promote a client-centered approach, empowering participants to make informed choices.
- Navigate, influence, and negotiate with public and community-based services to align with NDIS plans.
- Develop a professional network for optimal participant care in collaboration with ADS and other allied health services.
- Prepare for and follow up on NDIA plan meetings, assisting participants in negotiating plans according to their goals.
- Ensure timely development and implementation of participants' intervention plans.
- Build participant and family capacity to understand and navigate service systems.
- Facilitate discharge planning in consultation with healthcare professionals.
- Provide expert advice and consultation on the evolving NDIS environment.
- Support participants, families, and carers in developing resilience within the participant's network.
- Assist in the review of plans, addressing changes in circumstances or insufficient support.

**Job Types**: Full-time, Part-time

**Salary**: $60,000.00 - $108,656.94 per year

Schedule:

- Monday to Friday

Supplemental pay types:
