**Key Responsibilities**

The Care Coordinator will be responsible for:

  • Providing individualized support to participants with disabilities, including personal care, domestic support, and community engagement.
  • Developing and implementing personalized plans to meet the unique needs and goals of each participant.
  • Collaborating with other service providers, family members, and caregivers to ensure seamless support and coordination.
  • Maintaining accurate records and documentation, adhering to relevant policies and procedures.
  • Participating in regular team meetings and contributing to ongoing quality improvement initiatives.

**Requirements and Qualifications**

To be considered for this exciting opportunity, you will need:

  • A minimum of 2 years' experience working in a disability support or related field.
  • Relevant qualifications, such as Certificate III in Individual Support or equivalent.
  • Demonstrated knowledge of disability support principles, including Choice and Control and Person-centred approaches.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
  • Ability to maintain confidentiality and adhere to strict privacy standards.
