**About Us**
Palliative Care South East (PCSE) provides palliative care in the south east metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support at home to improve the quality of life of individuals, carers and families. Our specialist palliative care services are provided to people of all ages, free of charge, in the Local Government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.
**The Role**
Palliative Care South East is seeking a dedicated and empathetic Project Coordinator for a 6-month fixed-term contract (4 days a week). The role involves leading an initiative aimed at understanding the needs and concerns of current carers within our palliative care service. The primary focus of this project is to collaborate closely with carers, identifying their key priorities and challenges, and conducting a comprehensive gaps analysis to identify areas where support can be improved, and devise strategies to address these gaps effectively.
**Key Responsibilities**
- Facilitate discussions, interviews, and surveys to gather insights into carers' experiences, needs, and preferences.
- Conduct a thorough gaps analysis to identify areas where current support may be lacking or could be improved.
- Collaborate with internal stakeholders, including healthcare professionals and support staff, to develop strategies and initiatives to address identified gaps.
- Coordinate and implement pilot programs or interventions aimed at better supporting carers based on research findings.
- Provide regular updates and reports to management on the progress of the project, including key findings, challenges, and proposed solutions.
- Support the development of educational materials and resources for carers to enhance their understanding of palliative care and available support services.
**Requirements**:
- Proven experience in project coordination, particularly within healthcare or community services settings delivering clear outcomes
- Strong interpersonal and communication skills, with the ability to engage effectively with diverse stakeholders, including carers, healthcare professionals, and management.
- Clear communication capabilities including, empathy and understanding of the challenges faced by carers in the context of palliative care.
- Excellent organisational and time-management skills, with the ability to manage multiple tasks and priorities simultaneously.
- Experience in conducting qualitative research, interviews and surveys, and outcome reports
- Knowledge of palliative care principles and services is an advantage.
- Flexibility to adapt to changing priorities and a willingness to work collaboratively as part of a multidisciplinary team.
