- The Dementia Support Coach will facilitate on-the-ground support, implement relationship-based support strategies, model and coach with clients, carers and care staff
- Work collaboratively with peers to develop the skills and knowledge of aged care staff and carers to understand the person living with dementia and develop best-practice support and care
- Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required
- Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required
- Strong organisational abilities, including effective time management and the ability to prioritise in a busy work environment
- The ability to work both independently and as part of a team
- Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the provision of exceptional service to internal and external customers
- A current driver's licence and safe driving record
- Willingness to travel and work intrastate and interstate to support the delivery of DC services
- Qualification in relevant field; Allied Health Assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar (highly desirable)
