Support Coach

As a Support Coach, you will have the rewarding opportunity to support individuals living with dementia who are transitioning from hospital settings back to their homes or into residential care. Your role will be instrumental in facilitating on-the-ground support and implementing relationship-based strategies for clients, carers, and care staff.

Key Responsibilities:
  • Implement coaching techniques and facilitate on-the-ground support for clients, carers, and care staff.
  • Collaborate with peers to develop the skills and knowledge of aged care staff and carers to understand the individual living with dementia and provide best-practice support and care.
Essential Skills:
  • Strong organisational abilities and effective time management.
  • Ability to work independently and as part of a team, demonstrated commitment to excellence in customer service principles including conflict resolution.
  • Current driver's licence and safe driving record, willingness to travel and work intrastate and interstate to support the delivery of services.
