Job Title: Dementia Support Coach

This role presents an opportunity to be involved in a pioneering initiative focused on dementia support. As a Dementia Support Coach, you will work closely with individuals living with dementia who are transitioning from hospital settings back to their homes or into residential care.

Key Responsibilities:
  • On-the-ground Support and Coaching
  • You will facilitate effective support strategies and model coaching techniques with clients, carers, and care staff to develop best-practice care and support plans.
  • Support the implementation of these plans on the ground, utilising modelling and coaching methods.
Requirements:
  • Strong Organisational Skills
  • Able to work both independently and as part of a team.
  • Demonstrated commitment to delivering exceptional customer service principles.
  • A current driver's licence and safe driving record.
  • Willngness to travel and work across different regions to support service delivery.
  • Qualification in a relevant field such as Allied Health Assistant, Diversional Therapy, or Aged Care is required.
