What you will be doing:


An exciting opportunity has become available for a talented Social Worker to join the Lemongrove Campus Team in a Temporary Full Time Position.

The NBMLHD Community Health Care Team (CHC), provides comprehensive multidisciplinary assessment and co-ordinated care to ensure that clients with chronic medical conditions or complex care needs are treated and maintained at their optimal level of function and independence in the community.


In this position, you will provide Social Work services within a multidisciplinary approach to clients and/or carers referred to the service.

This will be carried out in accordance with professional codes of ethics and the NSW Health Code of Conduct.

The Social Worker will manage a clinical caseload, providing a professional level of service which is commensurate with their years of experience.

Level 1 staff are newly qualified employees who will work under discipline specific professional supervision.

Level 2 staff are expected to be competent independent practitioners, with at least 3 years clinical experience, who can work under mínimal direct professional supervision.

An eligibility list may be created for future vacancies.

Selection Criteria to be Addressed:

  • Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and a commitment to maintaining a professional skill and knowledge base. Overseas qualifications must supply the AASW Overseas Assessment number.
  • Current Driver's Licence and willingness to use for work purpose travel.
  • Clinical skills in assessment, planning and evaluation of social work interventions with adult clients who have chronic and complex health conditions and the capacity to work with this client group in a community setting. (Level 1 staff are newly qualified employees with less than three years clinical experience, who will work under discipline specific professional supervision. Level 2 staff have at least three years clinical experience, and are expected to be competent independent practitioners, with who can work under mínimal direct supervision).
  • Understanding of the psychosocial impact and management of chronic health conditions, ageing and disability and knowledge of grief and bereavement theory, commensurate with years of experience.
  • Commitment to professional development and training and willingness to engage positively in clinical supervision and performance development processes.
  • Knowledge of and commitment to evidence based practice and the ability to demonstrate how evidence based practice has been incorporated into your professional work.
  • Ability to work independently at a professional level as well as the ability to work productively as a member of a multidisciplinary team in a community health setting.
