About the role


We are currently seeking a dedicated Initial Assessment & Planning Worker & Case Manager - Barwon Region Prisons to ensure high quality provision of housing, information and referral services to persons at risk of homelessness, within and exiting custodial settings in the Barwon region, as well as metropolitan Melbourne.

Responsibilities include

  • Provide financial assistance from the Housing Establishment Fund (HEF)
  • Attend prisons within the Barwon region, to interview eligible individuals as identified through a referral system
  • Create transferable relationships and sustainable pathways out of homelessness
  • Incorporate assertive engagement and rapport building with clients
  • Foster services that are culturally sensitive to diverse client and community needs


This is a permanent full-time position, based in the HPP Office situated at the Western Metro Homelessness Service in Sunshine, you will also be required to outreach to the Barwon region correctional centres.

Salary and conditions are in accordance with SCHADS Level 5.

What you will have

  • Whilst not essential, a Tertiary qualification in Social Work or related community service field will be favourably regarded
  • Demonstrate knowledge of homelessness including causes and legislation
  • Experience working as a Case Manager and/or within homelessness sector
  • Applies homelessness approaches and practices when dealing with clients
  • Ability to understand the needs of individuals experiencing homelessness or at risk of homelessness
  • Knowledge of the Justice system and experience working within correctional settings is considered highly desirable
  • National Police Record Check is required
  • Current and Valid Working with Children's Check
  • Current VIC Drivers Licence
  • Ability to lift 5kg and carry items short distances due to instances where you may be required to take donated items to clients
