About the role


We are seeking a dedicated Initial Assessment & Planning Worker for our Housing Pathways Program You will ensure high-quality provision of housing and referral services to those at risk of homelessness.

Additionally, you will provide housing services and support to connect with the housing system for those in custodial care.

Responsibilities include

  • Assess immediate longterm housing needs to service users who are at risk of homelessness exiting designated facilities
  • Assist to resolve existing social housing tenancy and other issues on entry to or throughout a period of custodial care
  • Assess requests and provide financial assistance from the Housing Establishment Fund (HEF)
  • Attend prisons within metropolitan Melbourne region, to interview eligible individuals identified through a referral system
  • Provide occasional service provision at other custodial facilities managed by Housing Pathways Program as required
  • Foster services that are accessible and culturally sensitive to diverse client and community needs
This is fixed-term full-time position, contracted for 12 months, based in Sunshine, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

What you will have

  • Relevant Tertiary qualification in Social Work, Welfare or related field or relevant experience
  • Knowledge of homelessness including issues, philosophies and legislation
  • Applies homelessness models and practices when dealing with clients
  • Ability to respond to needs of individuals experiencing homelessness or at risk of homelessness
  • Knowledge of Justice system and experience working within correctional settings _(highly desirable)_
