Our Care Advisor is responsible for working in partnership with our clients in receipt of CHSP & STRC funding.

The main goal of our Care Advisor is to meet the objectives of the program with a focus on client choice.


Main role responsibilities include -

  • Assisting people to remain living at home for as long as possible
  • Enabling clients to have choice and flexibility in their care
  • Support clients in decisionmaking
  • Being responsible for and sensitive to client customs, beliefs and background
  • Wellness and enablement
  • Maintenance of independence and continuation of participation within the community
  • Maintain uptodate records in client files, assessments, care plans and documentation
  • Prioritise referrals
  • Maintaining client budgets
  • Assessment and identification of client goals
  • Liaising with service providers

What you will bring -
A relevant record of study related to Health, Aged Care or a similar industry is desired.

  • Experience in care management, ideally in Aged Care.
  • Current Victorian Driver's license
  • Current Police Check & NDIS clearance or willingness to obtain
  • Ability to work within budgetary KPIs.
  • Understand and promote a client centred approach.
  • Covid19 vaccinations (3 doses) and willingness to obtain a flu vaccination
