We have a new opportunity for an
End of Life / Palliative Care Coordinator to support primary care coordination between general practice and RACFs in order to improve the experience of the residents by coordinating GP input, RACF staff, external support services, as well as Residents and Family members, to address the current and future needs of the resident.


About the role:

  • Coordinate case conferencing within the RACF aided by the promotion of MBS items.
  • Develop systems and processes to ensure primary care coordination is a sustainable process for general practice and RACFs following completion of this pilot.
  • Engage GPs and RACF staff in quality improvement initiatives leading to better resident health outcomes.
  • Support advanced care planning for residents in the RACF through discussion with residents, families and GPs.
  • Implement evidence based clinical assessment tools to identify palliative care needs and support appropriate care planning including escalation plan where necessary.
  • Deliver inhouse education/coordinate education to build palliative care capacity within the RACF for all staff.
  • Attend Department of Health and Age Care community of practice meetings.
  • Undertake quality improvement activities aligned with the outcomes of this project and reflective of The Aged Care Quality Standards.
  • Undertake collaborative sessions with local palliative care services and other case coordinators.
  • Adopt the University of Wollongong Australia Palliative Aged Care Outcomes Program (PACOP) and appropriate tools associated with this pilot.

About you:


You're passionate about working collaboratively with other health professionals to improve care outcomes and bring excellent clinical assessment, diagnosis and treatment planning skills particular to Palliative and End of Life Care including advanced care planning.

In addition to a current, unrestricted AHPRA registration you will bring:

  • Significant palliative care experience
  • Demonstrated ability to work independently
  • Excellent interpersonal skills and communication, both written and verbal
  • A sound understanding and commitment to the personcentred approach in aged care
  • Interest in developing others and pursuing own development
  • Ability to learn from experiences and share them
  • Good understanding of evidencebased practice and critical reasoning

Desirable: A tertiary qualification in Palliative care e.g. Grad Cert/Diploma
