SummaryMental Health in Aged Care, to identify the complex and varied challenges faced by someone entering care. The program highlights opportunities for aged care facilities to offer additional support to people in the transition period from their own home into a residential aged care facility and the months following. Mental Health in Aged Care funded through Gippsland Primary Health Network and program ‘PLACE’ delivered by Wellways. The Mental Health in Aged Care ‘PLACE’ model is focused on transition into Residential Aged Care Facilities (RACFs) using trained peers and skilled transition support workers to facilitate the emotional journey and
address the practical issues in transition. Mental Health in Aged Care will compliment current RACF’s programs and services through the addition
of specific mental health psychoeducation programs, the development of peer leaders to support mental health outcomes, and the links to family support and education. Under the general direction of the Team Leader, this role will be responsible for providing the day-to-day wellbeing sessions for Gippsland. The Mental Health Wellbeing Workers will be responsible for the following undertake comprehensive assessment and triaging, undertake (typically) a face to face assessment session with the individual, develop a stepped care plan, make next appointment. provide face-to-face therapy for residents with mild- moderate anxiety and/or depression, including some complex cases.
offer reflection, clarification, outcome measures (clinical tools), risk strategy implementation. guidance, revision, make the next appointment.
