Community Support Incorporated is a leading provider of home care services in the disability and aged care sectors, as well as support coordination. We have been providing services in the community for over 30 years across South Australia and provide a professional and collaborative work environment with a clear focus on person-centred care for our clients.

**About you and the role**
As part of the Client Service Coordination team, the focus for this role is to draw on the incumbents' service delivery knowledge and expertise to support potential and new clients to understand their support options.
The Client Engagement Officer will then work with the client to design a Support Plan to achieve the clients' desired goals.
The Client Engagement Officer also works with the Client Service Coordination Team to develop and embed service design principles and resources across the client journey.
**Key experience required for the role**:
- Experience in a direct client service delivery role
- Up to date knowledge of the health and/or community services sector
- Participation in and contribution to improvement and / or change projects
- Experience in the not for profit / community sector, particularly agreements
- Care Coordination / Case Management desirable
- Passion for helping and supporting those who are living with a disability and are vulnerable.
- Previous experience in prioritising and managing workload within a busy environment.
- Highly developed interpersonal/communication (written and oral), analytical skills and problem-solving skills.
- Ability to work autonomously and in a team environment.
