Job Description

The Care Finder program was announced in the 2021-22 Commonwealth Budget as part of the Connecting Senior Australians to Aged Care Services measure. Care Finders will:
- Provide specialist and intensive assistance help people to understand and access aged care and connect with other relevant supports in the community; and
- Specifically target people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the Mitchell and Whittlesea shire
- Provide assertive outreach while building engagement and rapport with potential clients
- Guide clients through the assessment process and assist in obtaining Aged Care supports and services needed
Desired Skills and Experience
**Bring your great, everyday**
To be considered for this rewarding role you will have:
- Tertiary qualification in Human Services/Social Work or related profession and or previous experience in needs assessment, care planning and/or case management
- Extensive knowledge of the aged care, health and community services system and regulatory environment
- Experience in working with older people and their families/carers from a range of backgrounds, and an understanding of the barriers they may experience in accessing aged care services
- The ability to work collaboratively and autonomously and build effective relationships
- Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client focused services.
- Computer literacy and the ability to effectively manage a client data base
- High level problem solving, exceptional customer service skills and analytical skills
- Excellent IT skills, including Microsoft Office suite
- Knowledge of Carelink + desirable
- Full Victorian Driver's Licence
