We are seeking experienced professionals to join our team as part of the Care Finder program. This initiative aims to provide specialist assistance to people requiring aged care services, connecting them with relevant supports in the community.

The ideal candidate will have a tertiary qualification in Human Services or Social Work, and/or previous experience in needs assessment, care planning and/or case management. They will also have extensive knowledge of the aged care, health and community services system and regulatory environment.
