**Patient Flow Coordinator - Team Leader**

- An exciting opportunity exists for a Registered Nurse or Allied Health Professional who has experience in facilitating safe and effective discharge planning for patients in the acute hospital setting.
**About the role**:
RMH has a new team dedicated to facilitating the progression of care and patient flow through the hospital, for patients with an Estimated Discharge Date in the coming days and a 'routine' discharge destination. This position will lead a group of Patient Flow Coordinators.
- You will:
- Maintain a clinical caseload and be responsible for the day-to-day operations of the Patient Flow Coordinator team.
- Report to the Manager Access and Patient Flow
- Ensure adequate resources are allocated to areas of greatest need
- Lead by example in improving patient flow throughout the hospital.
- Liaise closely with key stakeholders across the hospital, including the Access team, Transit Lounge, ward NUMs/ANUMs and the Transport Manager.
- May be required to work some weekends and flexible shift patterns will be available depending on ward needs.

- Current professional registration with the Australian Health Practitioner Regulation Agency or other allied health professional association (where applicable to the profession)
- High level of clinical competence and expertise in specified area.
- Understanding of safe and timely discharge principles.
- Ability to lead a team and modelling of interdisciplinary practice.
- Effective interpersonal and communication skills both written and verbal (including conflict resolution and negotiation).
- Flexibility to adapt to different clinical wards and units to support the Patient Flow Coordinator and broader Access teams.
**Your Opportunity**:
- In collaboration with the multidisciplinary team, identify patients in advance of Estimated Discharge Date and address or escalate any barriers to discharge.
- Contribute to an improvement in the RMH Access and Flow KPIs that include NEAT, AV offload and reduced elective surgery hospital initiated postponements.
- Provide strategic and operational leadership of the Patient Flow Coordinator team.
- Foster positive team and workplace culture.
- Attend Tier 2 huddles and liaise closely with the Access team, Transit Lounge team and Transport Manager and other key internal stakeholders.
- Timely and appropriate escalation of pertinent patient flow issues via the agreed pathway.
- Demonstrate a multi-disciplinary health care approach that achieves safe and timely discharge for patients.
- Contribute to the evaluation of the service.
- Ensure patients, families and carers are well informed and ready to support discharge plans.
- Identify appropriate patients and facilitate discharge via the Transit Lounge and Criteria led Discharge.
- Facilitate regular team meetings and upskilling of team members.
- Initiate and drive a coordinated team approach to patient care.
- Maintain relationships with key stakeholders external to the RMH, such as transport providers, acute hospitals and residential aged care facilities.
