This position is responsible for providing specialist Social Work services to patients, clients, consumers and their family / carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above Social Work service to Renal Supportive Care clients at clinics in Wollongong, Shellharbour, Shoalhaven and Milton Hospital and in the community. The position works as part of a multidisciplinary team and provides clinical supervision to Level 1-2 Social Work staff and university students. The position provides advice and education to both Social Work and hospital staff on psychosocial issues in the Renal Medicine clinical area.
