About the role
This is an exciting opportunity to join Better Support Solutions' team as an NDIS Service Coordinator (RN) in our Parramatta, NSW office.
You will play a crucial part in providing high-quality care and support to our clients with disabilities, helping them achieve their goals and live more independent lives.
Full-time and part-time positions available – choose the hours that work for you.
What you'll be doing
Manage daily operations across participants
Prepare in-home / SIL care assessments and draft care plans to best meet client requirements
Ensure care aligns with NDIS goals and practice standards and client matching
Lead and mentor support staff to work effectively with complex needs
Maintain rosters and manage workforce performance
Uphold compliance with restrictive practices and Quality & Safeguards Commission guidelines
Mentor and support staff of the team
Drive performance reviews and staff development plans
What we're looking for
Current registration as a Registered Nurse (Division 1) with AHPRA with a strong understanding of the NDIS and disability support services
At least 2 years' experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS service coordinator
Excellent communication and interpersonal skills, with the ability to build trust and collaborate effectively with participants
Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for participants
Proficient in managing complex caseloads and maintaining accurate records and documentation
Commitment to delivering high-quality, person-centred care and support
Willingness to travel within the local area to visit participants in their homes or the community
