**Job Title**: Support and Outreach Coordinator - Wetherill Park, NSW

**Hours**: Part-Time, 10 hours per week

**About the Role**: We are seeking a compassionate and dedicated individual to join our team as a Support and Outreach Coordinator. This part-time role is ideal for someone passionate about providing personal care and community support to a 15-year-old with cerebral palsy and autism. You will play a crucial role in ensuring the participant's well-being while also engaging in outreach activities to enhance service awareness within the community.

**Key Responsibilities**:

- Provide 1:1 support during morning routines, including assisting the participant with getting dressed and preparing for school.
- Offer personal care services, such as feeding, manual handling, and hygiene assistance.
- Facilitate community access on Saturdays, operating a modified vehicle to ensure safe transportation.
- Support the participant in managing behaviors of concern, ensuring her physical and emotional well-being.
- Engage in outreach activities to promote awareness of services, including localized marketing and community engagement.
- Collaborate with the participant's family and our team to develop and implement individualized support strategies.
- Attend relevant training sessions, including manual handling and ASD Level 2 training.
- Participate in a meet and greet with the participant and her family to ensure a good fit before commencing the role.

**Qualifications & Requirements**:

- Must be a female aged mid-20s to 40s (as per client's preference).
- 100 Points of Identification.
- Valid Driver's License with access to a reliable vehicle.
- Car Insurance and Registration.
- NDIS Workers Screening Clearance.
- NDIS Workers Orientation Module Certificate.
- Police Check.
- Working With Children Check (WWCC).
- First Aid and CPR Certification.
- NDIS Quality, Safety, and You Module.
- Manual handling certification.
- Relevant experience in the disability sector, with a focus on support work and outreach.

**Behaviors of Concern**:

- Dislikes getting out of bed; may exhibit verbal behaviors of concern.
- May clench fists and growl when upset.
- Triggered by wind and unplanned changes to routine, which can cause significant distress.

**Training and Support**:

- Comprehensive training, including manual handling, ASD Level 2, and specific guidance from the participant's mother.
- Ongoing support from the team with opportunities for further professional development.

**Important Notes**:

- This is a part-time role
- A meet and greet with the participant and her family is required before the official contract is issued to ensure a good fit.

Pay: $35.00 - $38.00 per hour
