We specialise in providing assistance to NDIS participants with up to Level 3 complexity to navigate daily living/community access, Supported Independent Living (SIL) and 24/7 care across Melbourne, Western Victoria and NSW. Note: This position is for Campbellfield and Box Hill locations.

**Position Summary**

VHCC has built a comprehensive capacity-building program for individuals with disability (aged 16+) focused on fostering self-exploration, skill development, and meaningful learning experiences as our signature program. As the Program Coordinator, you will lead and manage the operations of VHCC's program alongside other Group Centre Activities across Campbellfield and Box Hill. Your role will be pivotal in delivering person-centered, high-quality services that empower participants to achieve their personal goals.

Reporting to the Operations Manager, you will be responsible for the coordination and service delivery of our Group Support activities & day programs, while driving cultural and clinical practices to ensure the delivery of high-quality services to our clients.

You will bring an in-depth understanding of the disability sector, particularly aligned with National Disability Insurance Scheme (NDIS), with a strong focus on coordinating and managing Day Programs, including both indoor and outdoor activities.

Your exceptional interpersonal skills will enable you to effectively negotiate individual service agreements and outcomes for clients, fostering collaborative relationships with families and key stakeholders.

Additionally, you will play a vital role in motivating and developing staff to achieve the desired results while driving the growth and expansion of our services.

**Key Responsibilities**
- Providing leadership and operational management to ensure smooth and efficient service delivery.
- Actively designing and implementing innovative group programs, including our signature Day Program and Social Support Programs.
- Tailoring services to ensure they are highly flexible and individualized, meeting the diverse needs of NDIS participants.
- Implement client-centered systems and processes that prioritize maximum choice and control for participants, respond swiftly to their evolving needs, and comply with risk management and WH&S principles.
- Develop and coordinate Day Programs, ensuring both indoor and outdoor activities are delivered at a high standard to NDIS participants.
- Train and support participants in building independent living skills through the VHCC Day Programs, promoting skill development and self-reliance.
- Ensure accurate NDIS claiming for Day Program activities in collaboration with the Claim Officer to maintain compliance and transparency.
- Collaborate with the digital marketing team to market and advertise Day Programs effectively, increasing visibility and participant engagement.
- Build awareness of programs by meeting with organizations and stakeholders, fostering collaborative partnerships and referrals.
- Document client outcomes and informed choices throughout the planning process to demonstrate a commitment to participant empowerment.
- Seek regular feedback from clients and staff regarding service delivery, using their insights to improve program effectiveness and participant satisfaction.
- Conduct individual assessments to enhance the value and impact of services provided, ensuring they align with client goals.
- Gather feedback during service plan reviews, develop feedback plans with clear targets and timelines, regularly review progress, and adjust services accordingly.
- Empower clients to engage in meaningful roles within their community and the organization, fostering social inclusion and personal growth.
- Contribute to team effectiveness by prioritizing work, sharing information, and supporting collective outcome.

**Key Selection Criteria**
- Proven expertise in planning, designing, implementing, and evaluating programs for individuals with intellectual and physical disabilities.
- Financial acumen to effectively set, manage, and monitor operating budgets and capital expenditure, ensuring sustainable service delivery.
- Exceptional communication and stakeholder engagement skills, with the ability to build strong relationships with clients, families, community partners, and staff, both autonomously and within a team setting.
