We are seeking a passionate and dedicated individual to join our Support Coordination team as a Support Connector. The role involves working closely with individuals with disabilities to understand their goals and connect them with appropriate and sustainable supports, services, and social networks.
You will play a key role in supporting individuals to navigate the NDIS system, access services they need to achieve their goals, build their skills, increase their confidence, and assist them to participate more fully in their communities in ways that are meaningful.
The ideal candidate needs to be an excellent team player, as they will work closely with the Support Coordination team members. This role will appeal to a person who knows a bit about the NDIS, is a good researcher and may aspire to be a Support Coordinator.
The role includes travelling to our Sunshine Coast office once a month.
This is a permanent part-time role, between 22.5 to 24 hours per week, to be negotiated with the successful candidate.
Key Responsibilities
Working with participants and their support networks to determine how their funds will be most appropriately spent
Empowering individuals and their families to manage some or all supports within their NDIS plan
Supporting and working with individuals to find suitable supports and services that align with their goals and aspirations
Researching and managing a range of therapy services and support providers, including coordinating the implementation of service agreements
Monitoring, supporting, tracking and reviewing individuals' NDIS plans, support providers and goals
Establishing and maintaining a professional relationship with stakeholders, such as NDIS service providers, allied health professionals, NDIA, Queensland Health, Office of the Public Guardian, etc
Assisting support coordinators with NDIS plan documentation, including plan reviews
Advocating on behalf of individuals to ensure their needs and preferences are understood and implemented
Ideal Candidate
Excellent written and verbal communication and social skills, including experience in recording activity notes
A commitment to the full participation and inclusion of people with disability within the community
High-level of organisational, time management and planning skills
Creative problem-solving skills
Strong research skills
Ability to think creatively, be self-motivated, work unsupervised and in a team
Excellent listening skills
Proficient computer skills
Ability to create and maintain lasting professional relationships
