**Position Summary**

DIS participants to implement their plans, build their capacity and navigate complex service systems.

**Key Responsibilities**
- Provide personalised and goal-oriented support coordination to NDIS participants
- Assist participants in understanding and activating their NDIS plans
- Connect participants to mainstream, informal and funded supports
- Monitor progress toward goals and ensure service delivery aligns with participant needs
- Build participant capacity for self-advocacy and independence
- Coordinate multidisciplinary support and resolve service barriers
- Prepare case notes, progress reports, and maintain all required documentation
- Collaborate with internal teams and external stakeholders to provide integrated care
- Advocate for participants' rights and promote informed decision-making

**Essential Criteria**
- Minimum 5 years of experience in Support Coordination, case management, or allied health roles
- In-depth knowledge of the NDIS framework, funding streams and compliance
- Proven ability to support individuals with complex needs, including disability, psychosocial issues, or dual diagnoses
- Strong communication, advocacy and interpersonal skills
- Excellent time management and organisational skills
- Capacity to work both independently and within a team
- High level of proficiency in written documentation and digital record keeping
- Commitment to person-centred, trauma-informed and culturally responsive practice

**Qualifications and Requirements**
- Relevant qualification background in Social Work/Community Service/Psychology/Allied Health
- Understand the NDIS framework
- Have strong communication and problem-solving skills
- Demonstrate cultural sensitivity for effective support coordination
- Current NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
- Valid driver's license and access to a reliable, insured vehicle
- Proficiency in Microsoft Office and case management systems
