**About the Role**

**Key Responsibilities**
- Conduct comprehensive client assessments to determine care needs and develop individualized care plans.
- Coordinate and manage aged care services, including home care packages and community support programs.
- Liaise with clients, families, healthcare professionals, and service providers to ensure seamless care delivery.
- Monitor and review care plans regularly to ensure they meet the evolving needs of clients.
- Provide information and guidance to clients and their families on available aged care services and funding options.
- Maintain accurate client records and ensure compliance with relevant regulations and standards.
- Advocate for clients to ensure they receive the best possible care and support.
- Assist in crisis intervention and problem-solving when issues arise with care plans or service delivery.
- Stay updated with industry regulations, policies, and best practices in aged care.

**Skills & Experience**
- Relevant qualifications in Social Work, Nursing, Allied Health, or a related field.
- Minimum of 1 years' experience in case management within the aged care sector.
- Strong understanding of aged care legislation, home care packages, and My Aged Care framework.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to work independently and collaboratively within a team environment.
- Strong organizational and problem-solving abilities.
- Proficiency in Microsoft Office and client management systems.
- Current driver's license and willingness to travel for client visits (if required).
- Valid police check and working with vulnerable people check (or willingness to obtain).
- **Fluency in Arabic is essential for this role.**

**How to Apply**

Pay: $33.00 - $38.00 per hour

Expected hours: 40 per week

Schedule:

- Monday to Friday
- Weekend availability
