Job Title: Transition Care Case ManagerAbout the Role

The Transition Care Program Case Manager plays a vital role in supporting patients transitioning from hospital care to safe home living. This temporary full-time position involves coordinating local program therapies, services, documentation, and communication to meet client needs within Transition Care guidelines.

The ideal candidate will have current NSW driving skills, tertiary qualifications in nursing or allied health, and experience in assessment, care planning, monitoring, and review of aged care clients. They will be responsible for engaging with NSW Health, SLHD, and Commonwealth Transition Care guidelines, ensuring compliance and coordination of transition programs.

Key Responsibilities
  • Coordinate local program therapies, services, documentation, and communication to meet client needs within Transition Care guidelines.
  • Support transition of inpatients to safe home living with goal-directed interventions for up to 12 weeks.
  • Ensure engagement with NSW Health, SLHD, and Commonwealth Transition Care guidelines, ensuring compliance.
