**About the role**

As a Case Worker in our Supporting Recovery Program, you will provide effective, flexible and holistic case management to clients who have experienced mental health concerns due to experiences of domestic violence, family violence, sexual violence which may include childhood sexual abuse. Working in a multidisciplinary team, you will undertake an initial assessment; provide high quality case management and referral pathways. You will have an in-depth understanding of domestic, family & sexual violence and relevant legislation providing outstanding service to our clients in a positive, respectful and culturally sensitive way.

**Duties**
- Intake triage and assessment.
- Provide case management to adults, young people and children to support recovery of mental health to victim survivors who have experienced domestic violence, family violence, sexual violence and or child sexual abuse.
- Engage clients using appropriate strengths-based assessment tools to develop, implement, monitor and review their plan and outcomes that identify strengths as well as areas of vulnerability, ensuring clients are actively engaged in the process.
- Facilitate health and wellbeing groups to promote connections and reduce social isolation for clients.
- Maintain an excellent standard of documentation and client records including timely and accurate case notes.
- Conduct regular reviews to assess outcomes and barriers.
- Assess client outcomes using health & wellbeing tools.
- Work collaboratively with partners across the sector including housing, Centrelink and medical services.
- Represent the organisation at Interagency meetings and events.
- Work across a multitude of LGAs including Campbelltown, Liverpool and Fairfield as required.

**Skills and Requirements**

You will hold qualifications in Social Work, Social Science or related discipline with:

- At least 3 years' experience working with complex case management, ideally working with people who have experienced mental health concerns related to trauma.
- Experience supporting individuals and or families where domestic violence, family violence sexual violence or child sexual abuse has been identified.
- An understanding of trauma-responsive practice and a goal focused approach with the ability to work with clients through all stages of engagement.
- A clear understanding of child protection issues and mandatory reporting responsibilities
- An ability to navigate systems and to advocate in a way that provides better outcomes for clients.
- Excellent time management and organising skills to balance competing demands.
- Excellent written skills in relation to note taking and record keeping.
- Experience using technology and electronic databases.
- Ability to work independently and as part of a team and contribute to a positive team culture.
- Experience with the Safe & Together model is advantageous.
